Spore Buster proposes to expertise in the areas of spraying Sandi Laboratories patented sanitizer for
"NAME CONFIDENTIAL" Public School District. The project will provide proof of contamination, complete spraying of all rooms, spraying of ventilation system and final testing to demonstrate effectiveness of application. Work will commence on or about [Insert Date] and will occur at the

"NAME CONFIDENTIAL" Public School District reasonably performing its responsibilities and delivering its deliverables as specified herein.

Under this agreement Spore Buster will define, develop, and deliver to

- • Materials and Equipment to spray all contracted space.

| Assumptions |

The project is based upon this Statement of Work. If the assumptions stated are in error, the pricing is subject to change. Additions, deletions or changes to this Statement of Work will require either a change order or a new Statement of Work accompanied with appropriate pricing, schedule, or deliverable changes before any work affected by the changes will commence.
"NAME CONFIDENTIAL" Public School District acknowledges these assumptions as valid upon acceptance and signing of this Statement of Work. The following assumptions were made by Spore Buster in the preparation of the cost, schedule and resource estimates contained in this Statement of Work: • Work will be performed after normal working hours defined as after 5:00 p.m. to 5:00 a.m., Monday through Friday. Weekend or holiday work will be considered a change of scope and will be handled through the change order process
- • Installation of vent sprayer to be provided by Spore Buster and installed until spraying is complete.
- • All physical site preparation activity (i.e. moving computers, printers and other electronic equipment must be implemented before Spore Buster personnel arrive on site.

"NAME CONFIDENTIAL" Public School District by providing customer satisfaction survey with return response for Spore Buster continued improvement and feedback for Sanitizing Engineer performance monitoring.

The Project Management Office (PMO) will be headed by a Spore Buster Project Manager and staffed by a team of Spore Buster resources dedicated to fulfilling the terms of the Spore Buster/"NAME
CONFIDENTIAL" Public School District agreement. The Spore Buster team will be comprised of a project manager, Quality Manager, Sanitizing Consultant and Sanitizing Engineers trained in administration of sanitizing spray as required to complete the project defined in this Statement of Work.
Spore Buster PMO will:
- • Be provided on an as required basis to manage this project.
- • Represent the single source of planning, coordination, and management of the
"NAME CONFIDENTIAL" Public School District project for Spore Buster.

Using the assigned Spore Buster resources, Spore Buster Project Manager will be responsible for providing:
- • Identify "NAME CONFIDENTIAL" Public School District’s needs/objectives for sanitizing, review building plans to determine square footage per building for spraying, specify approaches to completing project , recommend preferred approaches, and secure approval of
"NAME CONFIDENTIAL" Public School District to proceed with an approach.
- • Planning & Scheduling - Generate checklist for tracking rooms, hallways and other agreed to areas for sanitization. Completed Statement of Work will include the checklists, identification of resource requirements (personnel), assignment of responsibilities, and identification of methods to achieve the planed objectives. Working with the combined Spore Buster and
"NAME CONFIDENTIAL" Public School District teams, the Spore Buster Project Manager will develop the scheduling strategy and periodic forecast for the
"NAME CONFIDENTIAL" Public School District.
- • Implementation - Provide responsibility for project implementation. On a proactive regular basis manage Spore Buster resources, and work closely with
"NAME CONFIDENTIAL" Public School District resources. Act as the single point of
"NAME CONFIDENTIAL" Public School District contact for all issues and work both with
"NAME CONFIDENTIAL" Public School District and within Spore Buster to resolve issues. Coordinate site activities. Monitor and report the status of contract deliverables and project milestones. Control changes and ensure continued progress toward project objectives. Review actual schedules, budgets, costs, and results against project plans. Manage exceptions and risks, and implement corrections to achieve project plans. Establish and maintain all project files including correspondence, change summaries, contracts, historical records, plans, and other project related information
- • Project Reviews - Define and conduct joint

"NAME CONFIDENTIAL" Public School District with respect to:
- • Accepting project deliverables
- • Direct contact with persons authorizing payment
- • Facility and meeting coordination at
"NAME CONFIDENTIAL" Public School District's site resources
- • Interfacing with Spore Buster to insure there is an efficient exchange of information and that important and timely decisions are made.
- • Provide "NAME CONFIDENTIAL" Public School District staff as required to facilitate equipment/material moving requirements
- • Insure access to rooms for sanitizing are available at scheduled times
- • Available for final acceptance of project and provide payment on completion.

Any changes to this Work Statement must be mutually agreed upon by both Spore Buster and
"NAME CONFIDENTIAL" Public School District. No verbal agreement between persons involved in the project will be binding on either Spore Buster or
"NAME CONFIDENTIAL" Public School District. Mutually acceptable changes in the scope of work and adjustments in schedule and price will be incorporated as a modification to this agreement or may become the basis of a new, follow-on agreement.
The
"NAME CONFIDENTIAL" Public School District Project Coordinator is designated as the authorized
"NAME CONFIDENTIAL" Public School District representative for making changes to this Statement of Work. The Spore Buster Project Manager is designated as the authorized Spore Buster representative for making changes to this Statement of Work.
Change requests that impact project scope must be submitted in writing using Spore Buster standard Change Request Form. (See Appendix B)

| Issue Resolution |

An Issue Resolution Form is submitted when individuals working on
"NAME CONFIDENTIAL" Public School District projects experience problems within a given area. The status of open issues and actions taken or planned for resolution are published via the Issue Status Report.
The Spore Buster PMO, utilizing the form in Appendix B is responsible for logging and tracking issues for all parties. When the Project Manager for any party deems it necessary, s/he may escalate an issue to her/his and/or all parties management for resolution help. Each organization shall designate an escalation manager. Spore Buster Project Manager is the primary contact for issue escalation. Each escalation manager will actively assist in defining and influencing the resolution, and further escalate to their management when/if executive assistance is required. The following activities will be performed for each issue:
- • Define the problem and decide that it should be handled as an issue
- • Evaluate the issue, estimate its impact on the project and potential financial exposure
- • Identify corrective action(s) and personnel responsible
- • Log and track the issue
- • Notify appropriate business management
- • Follow up until satisfactorily resolved

- • Schedule status
Other reporting mechanisms will be discussed and implemented upon mutual agreement between the Project Managers.

Spore Buster is acting as prime contractor for the sanitizing application, as described in this SOW. Questions or issues pertinent to this SOW or a Spore Buster subcontractor(s) will be communicated to the Spore Buster Project Manager. |

As prime contractor, Spore Buster may engage subcontractor(s) to provide project deliverables. This Statement of Work includes products and services to be delivered by subcontractor(s) under Spore Buster direction.
Spore Buster, as prime contractor, is the sole contact for
"NAME CONFIDENTIAL" Public School District questions and issues with subcontractor(s) for these services. Spore Buster will provide a common, single interface to the subcontractor to:
- • Solicit consulting and technical information
- • Resolve business and technical issues
- • Coordinate delivery schedules
- • Insure services are delivered in a satisfactory time frame
"NAME CONFIDENTIAL" Public School District agrees to conduct any subcontractor project-related communications through the Spore Buster Project Manager.

"NAME CONFIDENTIAL" Public School District agrees to respond, within two (2) business days (unless otherwise agreed to by both the Spore Buster Project Manager and the
"NAME CONFIDENTIAL" Public School District Project Coordinator), to requests for the following documentation, information, or assistance needed for the PROJECT. This includes, but is not limited to, providing:
- • Information needed for completing sanitizing application .
- • Skilled and knowledgeable "NAME
CONFIDENTIAL" Public School District personnel to assist Spore Buster in the project where required
"NAME CONFIDENTIAL" Public School District is responsible for the accuracy and completeness of all information it provides. If information is incomplete, incorrect or if information is uncovered during the course of the project which could not be reasonably anticipated by Spore Buster, any work required to correct problems created by the use of such incomplete or inaccurate information or any additional work required by the discovery of such unanticipated information, shall be treated as a
"NAME CONFIDENTIAL" Public School District requested change to the scope of the work and subject to the change order procedure.
"NAME CONFIDENTIAL" Public School District will notify Spore Buster in advance of potentially hazardous environments. |

Spore Buster is providing the services described herein as a complete project and therefore reserves the right to staff the PMO as they deem necessary and to replace personnel at our sole discretion except for the Project Manager. Any potential Project Managers must be reviewed and accepted by Spore Buster and
"NAME CONFIDENTIAL" Public School District. Spore Buster will initially staff as follows:

- • One Quality Manager
- • One Sanitizing Consultant

To ensure customers receive the best service possible under the Spore Buster Sanitizing Program, the Spore Buster Company is actively asking customers to evaluate the assistance they received. The assessment is being completed using a written survey which will be distributed at the conclusion of the services received.
In Question 1, the survey asks respondents to rate a series of statements which evaluate various aspects of the project including the pre-sanitizing evaluation, the Project Management Team, Sanitizing Engineering Team and Quality Manager as a whole. Question 2 asks respondents how they learned about the Spore Buster Sanitizing Program. Questions 3 and 4 invite respondents to suggest any other services they would like to see included as part of the Spore Buster Sanitizing Program and to share any additional comments or suggestions they may have about the program overall.

We will first evaluate the extent of your damage. We will tell you what parts of your building or home already have been affected by mold growth and where you can avoid mold problems through comprehensive drying. When mold growth is visible, or otherwise found to be present, we will help guide you though the remediation process. |

When the evaluation is completed we will develop a comprehensive project plan consistent with industry standards and applicable regulations. A program of drying, demolition, and measurement will be implemented. |

We will use state-of-the-art dehumidification and drying techniques to remove moisture, therefore, prohibiting mold growth. We will engineer a pressurized system with advanced HEPA filtration to contain air inside mold contaminated areas. We use the most advanced monitoring equipment during all aspects of recovery. |
